Starter Team Mom Update

Here is a list of the paperwork each player will need by the end of the first week of practice: copy of your child's birth certificate, recent photo, Dr's physical (approval to play) and NKYFL contract signed by player and parent. This form will be given to your child the first week of practice.

Please turn all paperwork in together, in an envelope, to your team mom.
